Village residents can supply the Communications Center with the names and telephone numbers of individuals authorized to act on their behalf in emergencies or to be notified when an emergency occurs. The Police Department or other authorized persons will use this information only in emergency situations.

Residents, who wish to participate in this program, should complete an Emergency Contact Form (PDF) and return it to the Village Communications Center for processing. This form is available from the Communications Center as well as online.
